The Oklahoma Highway Patrol is reporting a personal injury collision occurred December 8, 2015 at approximately 05:28am on 86th Street North two-tenths of a mile east Sheridan near Owasso, OK in Tulsa County.
A 1993 Ford Explorer was being driven by John A. Lewis Jr. age 51, of Sperry traveling eastbound on 86th Street North. A 2004 Chevrolet Avalanche driven was traveling west on 86th Street North by Patrick C. Hoffman age 30, of Owasso, OK. . Lewis went left of center and struck vehicle Hoffman head-on. Lewis then rolled 1/4 time coming to rest on driver side. He was was transported by Owasso fire to St John hospital in Tulsa, OK. Subject was admitted with head and trunk injuries. Hoffman was not injured.
Seatbelts were equipped and worn by all in both vehicles.
