Student’s: Please wear shorts or work out pants. Jeans and skirts are not appropriate.
Physical Forms: All physicals need to be completed on the Owasso Physical Examination and Parent Consent Form.
Forms: Can be printed from the Owasso Public Schools Web Site at www.owassops.org or at the link below.
Important: These forms need to be filled out by the student and their parents before you arrive for physicals.
