Owasso Athletic Physicals
Owasso Athletic Department will be hosting Athletic Physical Night on Tuesday May 10, 2016 at the Owasso High School Gym starting at 5:30. All student athletes are required to have a physical to participate during the 2016-2017 school year. Below you will find an a blank physical form. Student Athletes should show up to physicals with the front page already filled out. It is imperative that after the front page is completed, it is signed by both the parent/guardian and the student athlete.

Student Athletes: Please wear shorts or work out pants. Jeans and skirts are not appropriate. Physical Forms: All physicals need to be completed on the Owasso Physical Examination and Parent Consent Form.

Important: These forms need to be filled out by the student and their parents before you arrive for physicals.
